We support people to increase their independence and quality of life. With each client at the heart of this process, Integra works in partnership with the client and the multi-disciplinary team to develop an outcome-focused care package designed to deliver agreed placement goals.
We demonstrate success through supporting individuals to make timely progression to accommodation with lower support, or independent living.
Through the development of a client-centred outcomes measure and user surveys, we monitor the client's perspective of how successful we are.
We support individuals in their own homes. Some of these are residential care homes - where there are clear benefits to the individual from this type of care e.g. for a short period of time following discharge from hospital - and others live in their own properties with staff visiting them under domiciliary care arrangements.
Integra has 8 registered care homes across South and West Wales in Carmarthen, Llanelli, Swansea, RCT and Cardiff.
To promote effective transitions, we are registered to provide domiciliary care, which can assist clients with continuity of support and promote effective move-on.
Integra is an approved provider of supported accommodation services to Cardiff and Vale University LHB.
Integra welcomes referrals for men and women over 18 with a mental health diagnosis and/or an acquired brain injury.
We have residential care, supported accommodation and outreach services in Cardiff, Carmarthenshire, Neath, RCT and Swansea.
